Please note: this form should be used to notify us of the passing of a customer who held a investment, pension or life cover with us, we call these Financial Services products. If the customer did not hold any of these products, complete the form for General Insurance policies.
If the customer held both Financial Services products and General Insurance policies you only have to complete one form, we'll pass the details between departments.
